Thank you for your interest in applying for one or more of your company's e-bike brands to become an E-Bike Positive brand. A key part of this process is that we (BA) conduct a company-level audit to verify that your operations and products are safe, and that your e-bikes are UK road legal. 
 

1. The documents linked to below provide basic info and details of audit costs, and more detailed guidance for quality staff who may be preparing for the audit. If you have questions on any aspect, please just ask.
    
2. When you are ready to proceed to book the audit, a person authorised to enter agreements on behalf of your company should complete the details below. 
    
3. After you submit the form, we will issue an invoice and the auditor will contact you to arrange an audit visit date. He will also request evidence for existing certifications or similar which you may wish us to take into account, ahead of the visit. 
    
4. After the audit (and once the invoice has been paid), you will receive a copy of the audit report and, assuming satisfactory, your brand will be eligible to become an E-Bike Positive brand, listed on the E-Bike Positive website, normally for three years.
    
5. Otherwise, there will be opportunities to send any missing paperwork, rectify any issues, or undertake a re-audit, as appropriate.

To apply for an audit, please fill out the details below. Some fields may be pre-filled with information we have from your registration on this website. If anything is incorrect, please just mention that in the "notes" field at the end. 

First, please detail the organisation applying for this audit, and the main contact for audit purposes and (if different) invoicing contact details.

Please list below the brands you would like to have listed on the Register. If any of these brands are subject to a separate or different quality process, please note that and we will contact you for more details.  

If you are not the brand owner of any of these brands, please ensure that you have the brand owner's permission before making this application. 

Please now detail below (in as much detail as seems relevant) any existing evidence or certifications which you have at organisation level around your quality process or business practices (see the Guidance document), and which we may be able to take account of to cover off some elements of the Audit.

There is no need to list product testing evidence for particular models here - this is more about organisation or premises level certifications or approvals. 

Please give a brief overview of the organisation applying - whether you are a manufacturer, own brand importer or importer of a named brand, for example, and a brief overview of where the products are sourced and warehoused. It would also be helpful to know, for example, how long you have been supplying e-bikes and how many models are represented by each of your brands.   

The following questions are to give us an idea of the location and scale of your operation. Please enter the address of the UK facility where quality process are handled and which we should visit for the audit. For the premises size and number of employees on site, approximate numbers are fine.
